Registered agents and brokers assisting consumers to apply for and enroll in Marketplace Insurance coverage must document consumer consent prior to accessing or updating their Marketplace information. CMS does not prescribe the manner in which agents and brokers must document consent. Instead, there are different formats that may be acceptable for agents and brokers to use to document consumer consent, such as via a recorded phone call, text message, email, electronic document with digital signatures, physical document with wet signatures, etc.

Acceptance of these Terms and Conditions: Indicating your acceptance of the terms and conditions by filling out, completing, and self-enrolling in an application for health insurance within the online services, including self-checkout quoting engines of Health Sherpa, or the mobile application or your use of any service through the online services or mobile application Provided by insurancemarketplace.exchange constitutes your acceptance and consent for insurancemarketplace.exchange (Agency) and NPN19359177 (Agent) to enroll you in a health, life, home, business, or supplemental Insurance Marketplace Plan.

Federally Facilitated Marketplace Coverage

By submitting your information for the purpose of an online health insurance quote, or navigating insurance options on your own you will be redirected and have access to the Federal Health Insurance Marketplace through a platform called Health Sherpa, an online insurance quoting tool designed to have all of your health options in one place. 

 

You are not required to make a decision or purchase upon receiving your quote and you agree that insurancemarketplace.exchange (the agency) and NPN19359177 may contact you at the number or email you have provided in the quote, with the potential use of an autodialer.  A sample voice recording may be used if we cannot reach you in regard to your submitted PII request, to better assist you during the process.

insurancemarketplace.exchange is not the Federal Insurance Marketplace itself and is privately owned by American Policy Insurers LLC and by submitting any form on this site or affiliate site hosted by Insurance Marketplace Exchange, you also agree to give consent for said company to receive, review, disclose, and maintain your Personal Identification Information (PII) for the purpose to help you navigate, research, and apply for health insurance plans. 

 

You (the applicant) give my permission to insurancemarketplace.exchange (the agency) and NPN19359177 to serve as the insurance agent of record, or broker for yourself and household members, if applicable, for purposes of enrollment in a Qualified Health Plan offered on the Federally Facilitated Marketplace. By consenting to this agreement, you authorize the above-mentioned Agency and Agent to view and use the confidential information provided by me in writing, electronically, or by telephone only for the purposes of one or more of the following:

1. Searching for an existing Marketplace application, whether by self-exploration or with the help of one of our licensed agents/brokers. 

2. Completing an application for eligibility and enrollment in a Marketplace Qualified Health Plan or other government insurance affordability programs, such as Medicaid and CHIP or advance tax credits to help pay for Marketplace premiums.

3. Providing ongoing account maintenance and enrollment assistance, as needed. 

4. Responding to and assisting with inquiries submitted to insurancemarketplace.exchange regarding Marketplace applications and coverage options.

You, understand that the Agency and/or Agent will not use or share your personally identifiable information (PII) for any purposes other than those listed above. The Agency and/or Agent will ensure that your PII is kept private and safe when collecting, storing, and using your PII for the stated purposes above.

You, confirm that the information provided for entry on my Marketplace eligibility and enrollment application will be true to the best of my knowledge. You, understand that You do not have to share additional personal information about Your health with the Agency and/or Agent beyond what is required on the application for eligibility and enrollment purposes. You hereby, understand that your consent remains in effect until you revoke it, and you may revoke or modify my consent at any time in writing or by electronic communication to insurancemarketplace.exchange.
